Workman Injury Compensation Claims

It is important that Employers report all WORK-RELATED ACCIDENTS involving their workers to the Ministry of Manpower (MOM) – Work Injury Compensation Department and our Claims Department immediately upon such occurrence.

Employers are required to submit a WSH Incident Reporting available at MOM website for the reporting of Work-Related Accidents.

The deadlines for reporting the type of occurrence as stipulated by MOM is listed below:

What to report Reporting timeline
Where the accident results in the death of any employee

Notify the relevant commissioner as soon as possible

Submit an incident report within 10 days

Where the accident results in any one of the following:

• Outpatient / hospitalisation leave (more than 1 day)

• Light duty (more than 1 day)

• Death

• Occupational disease

Notify the relevant commissioner as soon as possible

For fatal accident: submit an incident report within 10 days from the accident

For non-fatal accidents: submit an incident report within 10 days from employer’s first notice of the accident

Where the self-employed person of public dies at workplace as a result of works done

Notify the relevant commissioner as soon as possible

Submit an incident report within 10 days

Where the self-employed person of public injuries at workplace as a result of works done Notify the relevant commissioner as soon as possible

** Failure to report a work-related accident is an offence which carries a fine of up to $5,000 for a first-time offence, and a fine of up to $10,000 and/or jail term of up to six(6) months for subsequent offences.

Documents Required

  1. Original Medical Bill(s), Receipt(s), Certificate(s)
  2. Copy of MOM’s iReport
  3. Copy of the NRIC (for Singaporeans / Singapore Permanent Resident Workers) or Work Permit (for Foreign Workers)
  4. Copies of the salary voucher for the past 12 months prior to the accident
  5. Contract Agreement between Insured and Sub-contractor(s) -> (If injured worker is not under the direct employment of the Insured)
  6. Copy of the Sub-Contractor’s insurance policy (if applicable) -> If injured worker is not under the direct employment of the Insured)
  7. Contract Value of this project (applicable for annual policy)
  8. Copies of CPF and Levy Statements (from) on policy commencement date (applicable for annual policy)
  9. Any other documents relating to this incident.

1. Claim Form, duly completed and signed 2. Work Permit
3. Finalised Medical Bills
4. Inpatient Discharge Summary
5. Day Surgery Authorisation Form / Day Surgery Discharge Summary (For Day Surgery Only)
6. Doctor’s Memo / Letter (if any)
7. Laboratory / Radiology Reports if applicable

Motor Claims

Accident reporting is compulsory no matter if the vehicle is damaged or not / whether you will arise a claim. Please make sure you filed the accident report at the authorised workshop within 24 hours or the next working day.

Bond Called

Should advise whether the worker has been repatriated. If yes, please let us have a copy of the special pass and flight ticket.

Otherwise, the following documents/information are required:

  1. Reason for the bond call
  2. What is the employee’s whereabouts?
  3. Does he/she have a valid special pass? If yes, please let us have a copy.
  4. If not, please obtain a special pass for the worker while staying in Singapore.

Travel Claims

Common Documents to be submitted for Travel Claims:

1. Original Claim Form
2. Copy of Passport showing duration of trip
3. Copy of air-ticket and boarding pass
4. Copy of Itinerary (if any)

Depending on which item you are claiming, please submit the supporting documents within 30 days upon the date of your return together with the above mentioned common documents.

Where applicable, all documents submitted to us must be translated into English by a certified translator.

List of supporting documents For Medical Expenses
● Medical bills and receipts (original)
● Medical reports and in-patient discharge summary (original)
● Telephone bills relating to emergency telephone charges to our emergency assistance company (original)
● Medical leave certificate(s) (copies)
● Police or road traffic accident report (copies)

For Personal Accident/ Permanent Disability

● Death certificate (certified true copy)
● Proof of relationship between deceased and claimant
● Coroner’s/ post-mortem/ autopsy and toxicology report (copies)
● Police report or road traffic accident report (copies)
● Grant of probate or letter of administration (copies)
● Photographs of the claimant’s injury (in amputation cases)

For Travel Inconveniences

● Death certificate (certified true copy)
● Booking invoice and receipts (originals)
● Proof of deposit and documents showing proof of insolvency of travel/ tour agency in Singapore
● Proof of relationship between claimant and injured, sick or deceased person
● Written confirmation from travel/ tour agency, airline or other transport operator, hotel and/ or any other relevant party whom you paid the non-refundable prepaid travelling expenses to (original)
● Written confirmation from travel/ tour agency, airline or other transport operator and/or any other relevant party stating the period of delay you had experienced, the reasons for the delay and any remedial actions taken (original)
● Written confirmation from travel/ tour agency, airline or other transport operator and/or any other relevant party stating the reason and the amount of refund (if any) (original)

For Personal Belongings

● Report issued by the police at or around the place of the loss or damage within 24 hours of occurrence (original)
● Report issued by the airline or other transport operator, hotel and/or any other relevant party on any property irregularity (original)
● Acknowledgement slip or confirmation from the airline or other transport operator stating the date and time the baggage was collected (original)
● Proof of ownership of the lost or damaged item (for example, the purchase receipts or invoices) (original). In the absence of the above, instruction manual, original packaging, bank statement and photographs may be considered.
● Photographs of the damaged items
● Written confirmation from manufacturer or authorised repairer of the damaged item, stating that the item is beyond economical repair (original)
● Receipts or invoices for repair and/or parts of the damaged item (original)

Other Benefits

● Rental vehicle agreement (copy)
● Police or road traffic accident report (copy)
● Invoice for payment of the excess or deductible under the motor insurance policy covering the rental vehicle during the rental period, and the official receipt showing payment thereof (original)
● Witness’ accounts or other relevant details from third parties
● Photographs in relation to the incident
● Details of any solicitor that you have instructed
● Any correspondence from third party and their legal representative (original/copy)
● Fire incident report for home contents (copy)
